
Supportive housing is permanent housing with onsite services for families and individuals with special needs. CHI supportive housing programs are located in Westchester, NY and serve formerly homeless individuals with severe mental illness.
Facilities
CHI operates two programs with a total of 42 units for single individuals. One program is located in an 81-unit multiple dwelling building of which 21 apartments with 22 beds are set-aside for supportive housing tenants. Another program is located in a 74-unit affordable housing complex for seniors over the age of 62 of which 20 apartments are set-aside for supportive housing tenants.
Services
CHI provides onsite case management and counseling for supportive tenants to ensure housing stability and healthy living. Case Managers meet with tenants biweekly to assess their health and mental health status, food and nutrition intake, hygiene habits, and their level engagement with recreational and social activities. In response to tenant needs, Case Managers remind tenants to meet with their doctors, maintain their medication regimen, and connect them to community-based health service. For tenants who are experiencing food insecurity, Case Managers link tenants with food services like Meals on Wheels, food pantries and soup kitchens, and counsel them on nutrition and healthy eating habits. Case Managers identify and coordinate onsite and offsite services, workshops, and other activities that promote independent living such as home maintenance and anti-hoarding practices, participation in and utilization of local amenities including libraries and parks, and engagement in social activities such as holiday celebrations, game nights, or movie nights.

CHI can only accept referrals from the Westchester County Department of Community Mental Health (DCMH) and the Westchester Continuum of Care. Individuals in Westchester can apply for supportive housing programs through the county’s Single Point of Access (SPOA) system.
